How Can GME Infrastructure Benefit Rural Communities?

The Client

A rural community hospital with a small Graduate Medical Education footprint partnered with Germane Solutions to strengthen its existing primary care residency program and position the organization for future GME growth. As the sole program within the institution, it represented an important component of the organization’s strategy to develop physicians who could serve the surrounding rural community and support long-term access to care.

The Challenge

The residency program was navigating a combination of leadership, educational, operational, and recruitment challenges that were beginning to affect the overall training environment. While two of the program’s rural training sites were operating effectively, a third site lacked the faculty engagement, clearly defined roles, and infrastructure necessary to consistently support resident education. At the same time, the program was experiencing difficulty with recruiting residents, downward trends in resident survey results, curriculum challenges, and concerns related to board performance.

The Solution

Germane Solutions partnered closely with the organization’s incoming Program Director and GME leadership to support a broader vision for program improvement. The engagement focused on strengthening leadership integration, enhancing the curriculum, improving the training environment, and developing stronger recruitment and retention strategies.

As part of this work, Germane Solutions evaluated the program’s existing training structure and supported the organization in consolidating training around the two sites best equipped to provide a strong educational experience. The third site was ultimately sunset after operational and financial review identified insufficient faculty participation that raised concerns about the viability of continuing training at that location.

The team also supported development of a more structured curriculum across the full three years of training and expanded community outreach efforts to strengthen relationships with regional medical schools. Together, these initiatives created a more cohesive program infrastructure while supporting the organization’s ability to attract trainees and develop physicians who could ultimately remain in the community.

The Germane Effect

By addressing challenges across leadership, curriculum, recruitment, and training infrastructure, Germane Solutions helped the organization build a stronger and more sustainable residency program. The engagement improved trainee experience, strengthened recruitment and Match performance, and created greater opportunity to retain physicians within the surrounding community. It also demonstrated the broader value of GME to the health system, increasing leadership interest in strategically expanding its GME footprint.
